2. With Credit Card by phone or fax. Please call your order or Fax it.
3. With a Purchase Order. Fax or mail the order and indicate the Purchase Order Number.

4. With a check. Send the order with a check for the amount of the order and include 10% for shipping. If ordering from Florida also include 6.5% for State Tax.
